Customising desktop menu in 4.6

I've tried adding entries to the desktop in 4.6, based on http://wiki.xfce.org/howto/customize-menu - indeed there were already files in .local/share/applications from 4.4. 

When I restart xfdesktop (xfdesktop --reload doesn't work), instead of showing up on the top above the applications (i.e. near File Manager and Web Browser), it shows up mixed in with the menu (i.e. "E-mail" shows up between "Development" and "Games submenus.)  Is there any way to get it to show up on the top?

I've added an entry with categories

Categories=X-XFCE;X-Xfce-Toplevel;
OnlyShowIn=XFCE;

like the other top-level applications, but they still do not show up after restarting xfdesktop (retsart option doesn't work, so I have to kill it and it restarts by itself).

I don't think the menu is being cached, because if I temporarily remove the existing desktop items and restart xfdektop, they disappear.  But it just won't seem to let me add new ones.  Any advice?
