Does anyone know how I can remove old custom commands that clutter up the 'Open with' dialog in Thunar?

Thunar custom actions are stored in ~/.config/Thunar/uca.xml so you could just delete that file if you wanted to remove all of them.
I haven't tried it, but I'm guessing that you could also edit that file if you wanted to remove specific custom actions.

Thanks for getting back to me so quickly.
I don't have a uca.xml file. I've grepped all files in .config/Thunar/ and none of them mention my old commands. After testing, it seems that the uca.xml file is added when you add a command via 'Edit' -> 'Configure custom actions'.
Instead, I added the commands by right clicking on a file, then going 'Open with' -> 'Open With Other Application' and then 'Use a custom command'. It took a few attempts to get the command right and now all the old variations are showing up in the 'Open With' dialog (all with the same name).

Found them.
They were in .local/share/applications with file names like userapp-gvim-MYKXVV.desktop, each of which was referenced in mimeapps.list and some in mimeinfo.cache.
(gvim being the command I was adding)

Just to clarify, did you delete the appropriate .desktop files and then also remove the references to those files from mimeapps.list?
