issue with stereo sound from headphone output

Output to the laptop speakers works fine, but output to the headphone jack is mono (so I hear half the music).  I've played with the alsamixer and pulse audio controls (after switching it in xfce-mixer), but that doesn't seem to be the problem: adjusting the volume of the headphone left or right channel simply adjusts the volume of the mono output.

Has anyone experienced anything like this?  Any thoughts?

Did you confirm it is not a hardware issue, maybe with the headphones? Try them on something else, try a different set on the computer.

Does the output screen of pulse audio volume control show both channels?

Yes, it's not the headphones.  I tried with another pair as well.  Both work fine with my mp3 player.

And yes, the pulse audio control does show both channels.
